	1.	A copy of the filed income tax returns e.g. P.N.D. 50, P.N.D. 51.
	2.	A copy of tax receipt.
	3.	A copy of company’s tax identification card.
	4.	A copy of certificate of incorporation issued by Ministry of Commerce.
	5.	Name, address, and type of business in English.
	6.	Other relevant documents, if any.
	7.	An authorization letter in case of the taxpayer authorizes other person to act on behalf.
